EMPLOYER TAX FORMS & PUBLICATIONS

 

 
  Employer Identification Number

Form SS-4-Form SS-4 is used in order to receive an EIN. The IRS uses the number to identify taxpayers that are required to file various business tax returns. EINs are used by employers, sole proprietors, corporations, partnerships, nonprofit associations, trusts, estates of decedents, government agencies, certain individuals, and other business entities.

Paying Employee Federal Taxes

Publication 15:Circular E, Employer’s Tax Guide (68 Pages) is an excellent start to ensuring that your employee’s taxes are properly paid.

All of the following forms must be ordered from the IRS by using Form 7018 or by calling 1-800-TAX-FORM. They are provided here for example purpose only. They cannot be printed from an office printer and used because the IRS uses special inks and paper to ensure that the forms are computer-readable.

Form 1096-Form 1096 must be sent and acts as the summary and “cover sheet” of the submittal of 1099 (and other information Forms).

Form 8109-B-Form 8109-B is used to record Federal Tax Deposits.

Form W-2-Employers must file Form W-2 for wages paid to each employee from whom:

  • Income, social security, or Medicare tax was withheld; or,
  • Income tax would have been withheld if the employee had claimed no more than one withholding allowance or had not claimed exemption from withholding on Form W-4.

Form W-3Anyone required to file Form W-2 must file Form W-3 to transmit Copy A of Forms W-2. Make a copy of Form W-3; keep it and Copy D (For Employers) of Forms W-2 with your records for 4 years. Be sure to use Form W-3 for the correct year.

Form W-4-To know how much income tax to withhold from employees’ wages, you should have a Form W-4, Employee’s Withholding Allowance Certificate, on file for each employee. Encourage your employees to file an updated Form W-4 for 2004, especially if they owed taxes or received a large refund when filing their 2003 tax return. Advise your employees to use the Withholding Calculator on the IRS website at www.irs.gov/individuals for help in determining how many withholding allowances to claim on their Form W-4.
